CHRONIC DISEASE MANAGEMENT COHORT
The RHP 9 & 10 Chronic Disease Management Cohort is comprised of providers who are actively implementing change ideas and process improvement activities in primary and specialty care. Members include, but are not limited to RHP 9 & 10 DSRIP/UC Providers (hospitals, local health departments, local mental health authorities, academic hospitals, and physician groups), other stakeholders, and the Anchor teams.
Cohort Purpose: The Cohort will seek opportunities to increase communication and more effectively promote shared learning across the regions and state around efforts that impact the management of chronic disease health outcomes of the patient populations served by the 1115 waiver. Topics for this cohort will be decided and rotated based on the cohort participant’s recommendations. Participants in this cohort will discuss and take action with an overall focus on expanding and sustaining improvement efforts already achieved and identifying potential next steps.
