Come see how University of Texas Southwestern Medical Center is using Medication Management to enhance patient care and outcomes through integrating pharmacists into the discharge process and how they are using technology to treat depression through Measurement Based Care in primary care clinics.
Medication Management
The Medication Management project strives to improve medication usage by patients of UT Southwestern University Hospitals through extensive medication counseling provided by pharmacists at discharge. As applicable, instruction is focused on medications with a narrow therapeutic window in order to ensure patients adequately understand the dietary and drug interactions that could significantly affect their care. The project has created an opportunity for pharmacists to be an enhanced asset to the healthcare team through the prevention of medication errors and/or adverse effects.
Depression Screening
The Depression Screening project has implemented an innovative technology, iPad-based software designed to:
-
increase access to depression treatment for Medicaid patients through routine screening, and
-
increase the quality of depression treatment for Medicaid patients through the integration of Measurement Based Care (MBC) in primary care clinics.
The concept of MBC for the treatment of depression includes: the systematic assessment of depressive symptoms, antidepressant treatment side effects, and antidepressant treatment adherence at defined intervals using validated rating scales. The Depression Screening project’s aim is to implement Vital Sign (6), a depression screening software, used to capture the data from the depression screenings. This software is used by both patients and the medical staff.
